Il est difficile d'obtenir des chiffres fiables sur le nombre d'adhérents des AA ou de ceux qui se sont sevrés de l'alcool grâce à ce programme, notamment parce que ces membres sont anonymes et faute d'obligation de s'inscrire auprès d'un bureau central. Toutefois, le chiffre de 10 millions, fondé sur des conversations avec des chercheurs spécialistes des AA, paraît raisonnable (mais invérifiable), au vu de sa longue histoire.

En psychologie, ce type de traitement – cibler les habitudes – est souvent désigné par une formule générique, celle de « thérapie cognitive comportementale », ou, à une époque antérieure, par celle de « prévention de la rechute » (PDF). Le PDF, couramment employé au sein de la communauté des thérapeutes, intègre souvent cinq techniques de base : (1) l'apprentissage, où le thérapeute explique la maladie au patient et lui apprend à identifier les symptômes ; (2) le suivi, où le patient se sert d'un journal pour suivre son comportement et les situations qui le déclenchent ; (3) la réaction alternative, où le patient cultive de nouvelles routines, comme des méthodes de relaxation, afin de compenser le comportement source de problème ; (4) repenser, où le thérapeute guide le patient pour qu'il réévalue sa vision des situations ; et (5) exposer, où le thérapeute aide le patient à s'exposer aux situations qui déclenchent ce comportement.

15 . Écrire sur les AA n'est jamais facile, car ce programme compte beaucoup de critiques et de soutiens, et les interprétations sur le pourquoi et le comment de son efficacité abondent. Dans un e-mail, par exemple, Lee Ann Kaskutas, directrice de recherches au sein du Alcohol Research Group, écrivait que les AA « fournissent une méthode pour s'attaquer aux habitudes entourant la consommation d'alcool ». Mais cela passe par les individus au sein des AA, et pas par le programme lui-même. Le programme s'attaque au problème de base, le sujet alcoolique, l'alcoolique égocentrique, spirituellement démuni. Il est vrai, écrit-elle, que les AA apportent des solutions aux habitudes des alcooliques, comme avec ces slogans : « Si vous avez envie de boire, allez prendre part à une réunion des AA », et « évitez les gens, les lieux et les choses qui ne sont pas fiables ». Mais, ajoute-t-elle, « Ces slogans ne constituent pas le programme. Le programme, ce sont les étapes. Les AA veulent aller plus loin que simplement traiter l'aspect boisson de l'habitude, et les fondateurs des AA soutiendraient que s'attaquer aux habitudes n'est qu'une demi-mesure qui ne sera pas très utile ; vous finirez par succomber à la boisson, à moins de changer des éléments plus fondamentaux ». En réponse à mes questions, Dufrene souligne par lettre que les méthodes comme celles employées avec Mandy – que l'on appelle « la formation simplifiée à l'inversion des habitudes » (FRH) – diffèrent parfois d'autres méthodes de FRH. « Je considère que le Renversement Simplifié des Habitudes est efficace pour réduire les habitudes (par exemple se tirer les mèches de cheveux, se ronger les ongles, sucer son pouce), les tics (moteurs et vocaux) et le bégaiement », écrit-il. Toutefois, d'autres pathologies peuvent exiger des formes de FRH plus intensives. « Les traitements efficaces contre la dépression, le tabagisme, l'addiction au jeu, etc., se rangent sous l'appellation générique de “Thérapie Cognitive Comportementale” », écrit Dufrene, en soulignant que le remplacement simplifié des habitudes est souvent sans efficacité contre ces problèmes, qui requièrent des interventions plus intensives.

8 . Dans un entretien de clarification, O'Neill souligne sa conviction que les bonus et les promotions ne doivent pas être liés à la sécurité au travail, pas plus qu'à l'honnêteté. Plutôt, la sécurité est une valeur à laquelle chaque employé d'Alcoa devrait adhérer, indépendamment de toute récompense. « C'est comme dire “Nous allons payer les gens davantage s'ils ne mentent pas”, ce qui laisse entendre qu'un peu de mensonge serait acceptable, car nous vous paierons moins », m'a-t-il dit. Toutefois, il faut souligner que, dans des entretiens avec d'autres dirigeants d'Alcoa de cette période, ceux-ci m'ont indiqué que tout le monde savait que les promotions n'étaient accessibles qu'aux employés qui manifestaient un engagement en faveur de la sécurité, et cette promesse de promotion servait de récompense, même contre l'intention d'O'Neill.

9 . Lors d'un entretien de clarification, O'Neill a clairement indiqué que le concept de « boucle de l'habitude » lui était inconnu à l'époque. Il ne pensait pas nécessairement que ces programmes tenaient lieu de critère aux habitudes mais, rétrospectivement, il reconnaît que ses efforts sont en phase avec des recherches plus récentes indiquant de quelle manière les habitudes organisationnelles se créent.
