Conclusão
Vivemos atualmente uma realidade na qual a obesidade é uma pandemia e impacta de forma multifatorial a fertilidade masculina.
A mudança no estilo de vida é recomendada como primeira linha de tratamento de homens
obesos e inférteis, associada ou não à terapia medicamentosa.
A cirurgia bariátrica ainda é o tratamento que oferece os melhores resultados para o controle
de peso na obesidade mórbida, atuando como importante opção adjuvante, especialmente quando há falha na manutenção da perda ponderal em face das medidas clínico-comportamentais.
No entanto, a cirurgia bariátrica como tratamento para infertilidade masculina não deve, até o
momento, ser indicada de forma universal, ainda que a maioria dos estudos estabeleça relação direta ou indireta do excesso de peso a diversos fatores que potencialmente influenciam negativamente na busca conjugal pela reprodução.
Dessa maneira, durante a abordagem clínica ao homem obeso e infértil, após
descartadas ou tratadas outras possíveis causas para a infertilidade não relacionadas com a obesidade, a perda de peso sustentada deve ser o objetivo a ser alcançado. Nesse cenário, a cirurgia bariátrica pode ser considerada visando-se à melhora dos fatores intermediários, por exemplo da SM, observando-se, no entanto, o possível impacto deletério desta sobre a qualidade do sêmen. Nesse sentido, o tratamento deve, portanto, ser multidisciplinar e individualizado a cada paciente.
